Before the Move: Assessments that Come to the Residents
After a commitment to move to an ActivCare community, we schedule an assessment with our Nurse Case Manager who will visit the prospective resident where they currently live. The comprehensive assessment provides an overview of physical health, cognitive abilities, and individual care needs. From this assessment, giving special attention to safety and quality of life, the ActivCare team develops a personalized plan of care.

Welcome to the Community
ActivCare communities are specially designed and built for those with memory loss. The home-like atmosphere is warm and welcoming for residents and their guests. Elevated ceilings and access to outdoor courtyards provide a bright and airy atmosphere. Colorful wayfinding murals allow residents to easily navigate their apartment. Families are invited to bring their own furniture for resident rooms, so that the resident feels at home.

Our Doctor Makes House Calls
Daily care and support are provided by a staff of highly trained and compassionate caregivers who are scheduled around the clock. Licensed nurse support is on-site 7 days a week. Each community partners with a Medical Director, typically a physician who specializes in geriatric care, who makes house calls to the residents who are their patients in the community.

Loving and Supportive Care
Empower Me, a company that provides therapy has a studio set up on-site. For residents who need physical, occupational, or speech therapies, those services can be ordered by the resident’s physician and can take place within the community.

An appointment with the mobile podiatrist and/or dentist that comes to the community to provide care can be scheduled for the resident. These fee-for-service options are convenient for families and allow the resident to receive care without having to leave.
